Forms for attorney representation, biometric services, fee payments, and general USCIS business. Includes G-28 (Appearance), G-1145 (E-Notification), G-1450 (Credit Card), and G-639 (FOIA Request).

G series forms cover general USCIS procedural matters. Form G-28 is the notice of entry of appearance — the formal document attorneys and accredited representatives file to represent a client before USCIS. Form G-1145 requests e-notification when USCIS accepts an application. Form G-1450 authorizes USCIS to charge filing fees to a credit card. Form G-639 is the Freedom of Information Act (FOIA) request for access to immigration records. Form G-325A is used for biometric information in certain cases. Form G-884 requests return of original documents submitted to USCIS. These procedural forms are essential companions to the substantive immigration applications.
